The assessment takes place in the family home with the child present, unless the family chooses another setting. Assessments must include a face-to-face meeting with the child and family, and will ideally implement the five steps of Deciding Together (P-02246): Step 1: Issues and Goals. OPTIONS for Community Living is a state-funded program. The program was created to provide the elderly, as well as adults with disabilities, home- and community-based service choices.
